Services

MINORS:

Minors who are eligible for PRP services must be 5-17 years of age, have medical assistance, receive mental health services from a physician or licensed mental health provider, and meet diagnostic criteria for a mental health diagnosis. After following a period of an assessment which evaluates the individual’s strengths and preferences, the rehabilitation counselor will work with the client and his/her parent guardian to implement individualized rehabilitation treatment goals. Minors will be seen at least three times a month face to face. Services are offered in various settings such as Timeless Choice’ facility (onsite); at residence, job, or another appropriate location in the community (offsite); or at a combination of the two (blended onsite and off-site).


Target areas may include, but are not limited to the following:

 Providing psycho-education to assist clients with understanding their diagnoses

 Medication management

 Development of conflict resolution skills

 Development of age-appropriate social skills/natural supports through increased opportunities for socialization

 Referrals to community resources identified

 Assisting the parent/guardian with scheduling and maintaining appointments for the child/adolescent

 Assistance with securing entitlements when eligible (i.e. SSI, SNAP, TCA... )

 Development of crisis management and wellness for self-care/recovery related skills

 Coordination of care with other providers/treatment team members (i.e. physicians, psychiatrists, therapists, school staff, substance abuse treatment providers etc…)

 Life skills training (i.e. grooming, hygiene, nutrition, self-administration of medication)

 Community awareness/safety skills in various environments 

 Assistance with identifying resources for clients and their families experiencing homelessness (Education on McKinney Vento Act)

 Crisis Intervention/Safety Planning

 Other areas of need as identified
